At Active Health Medical Center, we’re a team of dedicated healthcare professionals who listened to what patients were saying—and decided it was time to do something about it.
We saw a growing problem in the healthcare system: patients waiting weeks or even months for a first appointment, only to end up in urgent care or the ER when they needed help sooner. Even worse, many walk away from those visits with unexpected bills and little to no follow-up. That sense of defeat is all too common.
We knew patients deserved better. So we built a solution.

We provide Primary Care, Neurology, and Mental Health services through a combination of telehealth and in-person options. Our local, community-based approach means you’re always connected to someone who cares.

Frequently Asked Questions
While Active Health Medical Center, is not looking to replace insurance or current provider, we are looking to complete it. Your monthly membership covers all primary care services, and you can still use your insurance for any testing or radiology exams ordered. However, if you have Medicare and Medicaide due to regulations, we can not offer this service to you.
This combination of insurance can offer more comprehensive care while reducing out-of-pocket expenses.
NavCare offers benefits such as no co-pays or deductibles, more time with your provider, and easier access through direct communication methods like phone or text. This model allows for better provider-patient relationship and better outcomes while closing the gaps.
Your NavCare membership at Active Health is barrier-free access to your healthcare provider, same-day or next-day appointments, routine check-ups, chronic disease management, preventative care, and more. Our goal is to provide you with comprehensive care that meets your unique health needs without the barriers a of traditional insurance-based models. We also play a large role in helping you transition your treatment over to your providers office.
