Our Lutz Office

Active Health Medical Center maintains an administrative and clinical operations office in Lutz, Florida. 

This location supports scheduling, clinical oversight, laboratory coordination, and provider operations.

At this time, the Lutz office is not accepting in-office appointments. This is intentional as we reach our patient load and expanding outward.
Our current patient demand is overwhelmingly for in-home and virtual care, and our resources are

 dedicated to meeting patients where they are most comfortable and where they experience the least disruption to their day.

As the practice continues to grow, we will reassess the availability of limited in-office appointments.

 Until then, all new patient visits are offered as mobile or virtual services, allowing us to provide timely, 

accessible, convenient care 

without long waits or crowded lobbies.

We are not a luxury concierge service.
We are a mobile medical clinic designed to make quality care more accessible and convenient, especially for patients who:

  • Prefer not to leave home when ill

  • Have limited transportation

  • Have demanding work or family schedules

  • Simply value avoiding waiting rooms and crowded offices

How it works

How a home visit with Active Health works

Step 1 – Reach out
Contact our office through our website or main phone/text line. A team member will review your needs and service area and help determine the best type of visit (urgent, primary care, or follow-up).

Step 2 – Simple intake
We’ll send you secure forms to complete electronically before your visit. This allows your clinician to review your history, medications, and concerns in advance.

Step 3 – In-home visit
At your scheduled time, a licensed clinician arrives at your home, conducts a thorough assessment, discusses your concerns, and develops a plan of care. When appropriate, labs, testing, and prescriptions are coordinated during or shortly after the visit.

Step 4 – Follow-up & coordination
Results, treatment updates, and follow-up plans are communicated through our office. All calls and messages go through our central line so providers can stay focused on clinical care and documentation.

Please note: For privacy and safety reasons, personal provider phone numbers are not given out. All communication is coordinated through our main office contact.

This allows us to focus our resources on what patients are asking for most: safe, convenient care delivered where they are.Who we serve

Who is mobile care a good fit for?

Our mobile services are designed for patients who:

  • Prefer to avoid waiting rooms and urgent care lines

  • Work from home or have demanding schedules

  • Have young children, older family members, or caregiving responsibilities

  • Have mobility, transportation, or health limitations

  • Value a calm, unhurried visit in their own space

We care for patients from 6 months and up, and work with individuals, couples, and families who want a more convenient way to access traditional medical care.

Insurance and payment information

Active Health Medical Center currently operates as a mobile, out-of-network medical service. This model allows us to spend more time with each patient and reduce many of the delays and restrictions associated with traditional insurance-based care.

At this time, Medicare and Medicaid are unfortunately not approved for our services due to regulatory and contractual limitations. 

We are happy to provide a summary of your visit that you may submit to your commercial insurance plan, if applicable, for possible out-of-network reimbursement, depending on your policy.
